New Patients

Initial Visit

  1. All new patients are requested to fill out all four pages of the confidential Patient Health record.
  2. New patients should schedule a consultation with a doctor to discuss your health problems.
  3. The diagnostic chiropractic examination procedures help to determine if chiropractic care is appropriate for your condition.
  4. The Doctor will review x-rays (if they are brought with you), or take an x-ray to advise proper procedures.
  5. If your case requires immediate attention at this time, every effort will be made to administer temporary relief.
  6. You will be advised as to a time you can return for your "Report of Findings" when your doctor will inform you as to your examination results and whether or not your case has been accepted. If accepted, your recommended treatment program will be explained to you. You will also be advised concerning financial arrangements and insurance coverage, as appropriate. It is vital that your spouse, guardian or family member be present.
  7. Treatments will begin and continue as scheduled until your condition has been fully corrected or until maximum possible improvement has been obtained.

Payment Options

Most major insurance plans are accepted. Insurance carriers are looking for ways to promote wellness and good health, and First Choice Healthcare office staff is happy to work with you on filing your insurance as well as call for pre-certifications, if necessary. Co-payments and uninsured portions of your care are due at the time of service. For your convenience, First Choice Healthcare accepts cash, checks, Visa,  Mastercard, or Discover.
